NEXT SHIFT HOCKEY TRAINING

WAIVER AND RELEASE OF LIABILITY

By agreeing to these terms, I acknowledge and accept the following:

Assumption of Risk

I understand that participation in hockey training, including on ice instruction, skating, drills, scrimmages, off ice conditioning, and use of hockey equipment, involves physical exertion and inherent risks. These risks include, but are not limited to, falls, collisions, contact with other participants, pucks, sticks, boards, equipment related injuries, ice conditions, dehydration, muscle strains, sprains, and other serious injuries. I freely and voluntarily assume full responsibility for any risks, whether known or unknown, arising from participation in programs offered by Next Shift Hockey Training.

Release of Liability

In consideration of being permitted to participate, I hereby release and discharge Next Shift Hockey Training, its owners, directors, coaches, instructors, trainers, employees, independent contractors, agents, as well as any facility, rink, or property owners where training occurs, from any and all liability, claims, demands, or causes of action arising out of or related to participation in training sessions, classes, or events, whether caused by negligence or otherwise, to the fullest extent permitted by law.

Medical Authorization

I authorize Next Shift Hockey Training staff or facility personnel to provide first aid and, if necessary, obtain emergency medical treatment on my behalf or on behalf of my child. I understand that I am financially responsible for any medical care, treatment, or services required as a result of participation.

Health Certification

I affirm that the participant is physically fit to participate, has disclosed any relevant medical conditions, and will notify staff of any changes to health status that may affect safe participation.

Code of Conduct

I agree to follow all safety rules, coaching instructions, and facility guidelines set forth by Next Shift Hockey Trainingstaff. I understand that failure to comply may result in removal from participation without refund.

Media Release

I grant Next Shift Hockey Training permission to use photographs or video recordings taken during training sessions for coaching, instructional, marketing, or promotional purposes, unless I notify Next Shift Hockey Training in writing that I do not consent.

Acknowledgment of Understanding

I have carefully read and fully understand this waiver. I acknowledge that by agreeing electronically, this document serves as my legal signature. I understand that I am giving up certain legal rights, including the right to sue, and I am doing so voluntarily.
