Girls Session (Friday)

1. Assumption of Risk

I understand and acknowledge that ice hockey and goalie training involve inherent risks, including but not limited to falls, collisions, puck impact, skate‑related injuries, and other physical hazards. By participating in any on‑ice or off‑ice training session with Sibell Hockey, I voluntarily assume all risks of injury, whether foreseeable or not.

2. Release of Liability

In consideration of being allowed to participate in Sibell Hockey training sessions, I hereby release, waive, and discharge Sibell Hockey, its owners, coaches, staff, contractors, and affiliates from any and all liability for injuries, damages, or losses that may occur during or as a result of participation in training activities.

This release applies to all claims, including those arising from negligence, to the fullest extent permitted by law.

3. Medical Authorization

I certify that I am physically able to participate in goalie training and understand that Sibell Hockey does not provide medical insurance. I authorize Sibell Hockey staff to seek emergency medical care if necessary, and I accept full financial responsibility for any resulting costs.

4. Media Release

I grant Sibell Hockey permission to record, photograph, and/or film me (or my child) during training sessions. I authorize Sibell Hockey to use this media — including video clips, photos, and training footage — for promotional purposes such as social media, websites, and marketing materials.

I understand that all media will be used in good faith and without compensation.
