2025/2026 WAIVER
For Group: TC Goaltending
ACKNOWLEDGEMENT, LIABILITY WAIVER FOR PARTICIPANTS
The undersigned understand and acknowledge, or as parent/legal guardian of the Participant who is under the age of majority in the jurisdiction in which they reside, that there is potential risk involved in the training for, and participation in, any physical activity, including any event hosted, organized, administered, sponsored by, or otherwise involving, iPlayHockey. While rules and guidelines may reduce the risk associated with participation, the risk of serious illness, injury, and death does exist. The undersigned freely accept and acknowledge all risks, dangers and hazards resulting from participation in, or attendance at, any event hosted, organized or sponsored by iPlayHockey.

The undersigned is also aware and should discuss the participation of the Participant in any activity, program or event hosted, organized, sponsored by, or otherwise involving, iPlayHockey with the Participant's physician to determine the effect on the Participant's current health. As a Participant or parent/legal guardian of the Participant, the undersigned acknowledge their responsibility to ensure adequate medical personal health, dental and accident insurance coverage for the Participant, as well as protection of their personal possessions.

The undersigned hereby agree to release, remise, waive and forever discharge any and all past, present, and future losses, claims, actions, damages, liability, costs, or expenses of whatsoever kind or nature, against iPlayHockey, its coaches, officials, members, agents, directors, officers, employees, representatives, and other participants (all of whom are hereinafter collectively referred to as “Releasees”) with respect to participation in, or attendance at, any event hosted, organized or sponsored by, or otherwise involving, iPlayHockey, howsoever caused, including negligence of the Releasees. By signing this agreement, the undersigned is aware that they are waving certain rights they may have against the Releasees and that this release is binding on their heirs, executors, administrators, and assignees.

This waiver shall be governed and construed in accordance with the laws of the province of Ontario and the federal laws of Canada applicable therein, and the parties hereto expressly and irrevocably submit to the exclusive jurisdiction of the courts of the Province of Ontario.
