Rush Hockey – August 18-21st Back2Back’s – 2013/2014 Group

I, undersigned, hereby acknowledge and agree to the following terms as a condition of my/my players participation in Rush Hockey Development Inc. Assumption of Risk: I understand that participation in hockey involves inherent risks, including but not limited to serious injury or death. I voluntarily assume all risks associated with my participation in Rush Hockey Camps. Release of Liability: I hereby release, waive, and discharge the Rush Hockey Development Inc., its organizers, sponsors, coaches, officials, and any volunteers from any and all claims, liabilities, injuries, or damages that may arise from my participation in the league, whether caused by negligence or otherwise. Medical Authorization: In the event of an injury or medical emergency, I give permission for medical treatment and transport as deemed necessary by a Rush representative. I understand that I am responsible for my own medical expenses. Indemnification: I agree to indemnify and hold harmless Rush Hockey Development Inc. and its officials from any claims, actions, or damages arising out of my participation in the League. Media Release: I grant permission to Rush Hockey Development to use my image, likeness, or voice in any promotional materials or publications, including social media, without compensation. Governing Law: This waiver shall be governed by and construed in accordance with the laws of the province in which Rush Hockey Development operates.
