U8 Mite In-House Fall Registration

In consideration of the athletic training services and all services rendered to myself or of my child (hereinafter referred to as "client") by Elite Edge (hereinafter referred to as "coaches"), I hereby release and discharge the coaches, his agents, servants and employees as well as Elite Edge from all undersigned's heirs, executors, administrators, or assigns may have, or claims to have against the coaches, his successors, or assigns, for all personal injuries, known or unknown, and injuries to property, real or personal, caused by, or arising out of, the above-mentioned, services. In addition, the undersigned, on behalf of himself and the client, hereby assumes full responsibility for and risk of bodily injury; death or property damage due to the negligence of the trainer, his servants, agents or employees as well as Elite Edge, participating in the above-mentioned training services. The undersigned agrees to identify the trainer, his agents, servants and employees as well as Elite Edge Hockey from any loss, liability, damage or cost the coaches may incur due to the presence of the undersigned and/or the client in or on the training area whether caused by the self, coaches, or other players participating in the camp/lessons. The undersigned expressly agrees that the foregoing release waiver and indemnity agreement is intended to be as broad and inclusive as is permitted by the law of Pennsylvania, and that if any portion of it is held invalid, it is agree that the balance shall, notwithstanding, continue in full legal force and effect.

I also understand that while renting the use of the Elite Edge facility anywhere on the premise it is my responsibility and take full responsibility for any injuries or death that may occur on the Elite Edge premise inside or out for myself and or individuals that I invite for birthdays and or ice/gym rentals. 

During birthday parties it is known that ice skates are sharp and to improve everyone’s safety skates will be taken off and removed when in the lobby and party room. It is the  Birthday rentals (person renting Elite Edge for the birthday) responsibility to abide by these rules and to enforce them. As the birthday rental I take full responsibility for any injuries or death that may occur on the Elite Edge premise inside or out for myself and or individuals that I invite for birthdays and or ice/gym rentals.

I permit Elite Edge to use any gym, on-ice, or event videos and pictures for website, social media, and branding purposes taken of myself and or children in and during participation.

I have read this release, waiver of liability and indemnity agreement, and understand all terms. I execute it voluntarily and with full knowledge of its significance.
